*All above services are eligible for submission to insurance unless you are seen virtually and physically outside Minnesota at the time of service. Those telehealth services will not be eligible for submission to insurance and will be billed as a wellness visit.

While you are under Hive’s care…
Support Between Visits
While you are under Hive’s care, there may be times when you request additional support from your physical therapist outside of a scheduled appointment. We are happy to assist with these requests; however, some requests require dedicated clinical time.
Examples may include:
- Letters of medical necessity or documentation requests
- Exercise clarification or treatment guidance outside of a visit
- Care coordination with other healthcare providers
- Specialty projects such as ergonomic assessments or customized exercise programming
When provider time is required outside of a scheduled visit, it is billed at $55 per 15-minute increment. Brief administrative questions or simple logistical messages are typically addressed without charge. Many patients find it helpful to bring questions to their scheduled visits so they can receive the most thorough guidance.
For All Visits
Hive Therapy and Wellness is a cash-based practice, meaning full payment is due at the time of service. We are out-of-network with all insurers—however, we do accept HSA/FSA payments, offer discounted treatment packages, and provide superbills that you can submit to your insurance for out-of-network coverage.
When booking an appointment with Hive, a valid credit card will be required at the time of scheduling.
About Treatment and Diagnosis Codes
When starting treatment with Hive Therapy and Wellness, our providers will determine diagnosis and treatment codes that accurately reflect your treatment and plan of care. Diagnosis codes are decided at the time of your evaluation, and treatment codes are determined at each session. These are unable to be altered at any point during the remainder of your care. In other words, any billing related to these codes cannot be adjusted after they are entered into our system.
Therefore, please be sure to contact your insurance beforehand to discuss coverage, and have that information prepared ahead of time when booking your evaluation with Hive, in case you have questions about billing. The treatment codes utilized at Hive Therapy and Wellness include:
97163 – Physical Therapy Evaluation
97164 – Physical Therapy Re-evaluation
97110 – Therapeutic Exercise
97112 – Neuromuscular Re-education
97116 – Self Care / Home Management Training
97140 – Manual Therapy
97530 – Therapeutic Activity
97116 – Gait Training
*Cost per treatment code will depend on whether you opt to pay full price for the session or choose a treatment package.
Cancellations and
Missed Appointments Policy
Patients who fail to reschedule or cancel their appointment at least 24 hours prior to their appointment time will be charged in full for the cost of their visit. However, we are always able to switch to a video visit or phone visit to accommodate last minute changes.

Refund and Return Policy
At Hive Therapy and Wellness, all sales on services and service packages are considered final. This includes discounted package services.
Service packages or discounted plans remain valid for one year from the date of purchase. While packages are non-refundable and non-transferable, any unused visits can be saved for your own future use within that one-year
period.
Good Faith Estimate
The No Surprises Act allows patients who do not have or choose not to use insurance for their services (self-pay) the right to receive a Good Faith Estimate (GFE) in writing (either on paper or electronically).
